When Rosa’s son began to severely deteriorate, the current public framework forced her to watch his decline without support. She was left unguided, isolated, and entirely alone to navigate complex privacy barriers and rigid legal systems. Our Crisis-to-Care (C2C) framework ensures no family is ever abandoned in a crisis. We protect families by actively intervening upstream — long before a medical crisis is allowed to become a carceral catastrophe.
Pillar 1: The Family Peer Navigator
Families caught in a Severe Mental Illness (SMI) crisis are routinely isolated by a fragmented administrative ecosystem. C2C pairs them with a dedicated Family Peer Navigator — an expert caregiver who has personally navigated the front lines of severe brain illness. They provide immediate, trauma-informed accompaniment, break the agonizing isolation, and help caregivers execute a continuous strategy for community-based clinical engagement.
Pillar 2: The Family Medical Homebase
Families navigating severe mental illness often struggle to organize critical information across years of diagnoses, hospitalizations, medications, providers, and crisis experiences. Through Primary Record, C2C provides families with a secure, longitudinal medical homebase designed to help them organize health information, document important history, and better prepare for conversations with healthcare and community systems. AI-supported guidance helps families identify relevant information, understand available resources, and make more informed decisions while keeping the family at the center of care coordination.
