Dismantling $343B Neglect Tax

Architecting the transition from a "Standard of Neglect" to a National Standard of Care for no-fault brain disease.

THE BLUEPRINT FOR MEDICAL ACCOUNTABILITY

For decades, the United States has tolerated a "Standard of Neglect" for serious mental illness—a policy choice to wait for catastrophe before providing care. While we have world-class response systems for fires and heart attacks, for families living with no-fault brain diseases, there is no "911." Parents are left to navigate a clinical void where the "Right to be Sick" is prioritized over medical necessity.

The National Shattering Silence Coalition (NSSC) is providing the strategic roadmap to end this crisis. Our 2026 Impact Initiatives—including the Crisis to Care Pilot, the SMI Peer Alliance, our Beyond Stigma documentary series, and our State Director Mobilization—are designed in lockstep with this roadmap to establish and promote a National Standard of Care.

By securing the necessary support to activate these strategic pillars, we will move the focus from containment in jails to recovery in hospitals, securing the fundamental Right to be Well. The results of the current system are visible in our jails and on our streets; it is time to replace systemic abandonment with medical accountability.

THE ARCHITECTURE OF ACCOUNTABILITY: COMPARING STANDARDS

Standard of Neglect
National Standard of Care
✕ Handcuffs over Humanity Policing biological brain crises as criminal behavior.
✓ Medical Necessity Decisions driven by clinical pathology and anosognosia.
✕ Competency Cycling Containment without care: Warehousing the sick in jails with no medical treatment or stabilization.
✓ Continuous Clinical Pathways Psychiatric beds, structured outpatient programs, and supportive community housing necessary for long-term recovery.
✕ ER Boarding & Premature Discharge Treating emergencies as inconveniences; discharging patients before clinical stabilization to the streets.
✓ Systemic Accountability Ending the "street discharge" cycle by mandating a full continuum of medical restoration.
✕ The Pediatric Pipeline The revolving door to justice, streets, and graveyards: allowing childhood SED to deteriorate without medical intervention.
✓ Early Intervention Upstream clinical discovery to shut the pediatric pipeline and prevent the revolving door from ever opening.
✕ Systemic Abandonment Waiting for "Danger," resulting in permanent gray matter loss and preventable death.
✓ Restorative Mercy Lifesaving intervention to return a human being to their own reality—granting them true autonomy, dignity, and a life worth living.

The National Shattering Silence Coalition (NSSC) is turning decades of pain into systemic power. We are no longer simply documenting the "Standard of Neglect"—we are architecting its replacement, a national Standard of Care.

Our 2026 Strategic Roadmap represents a unified vision for medical accountability. By scaling the SMI Peer Alliance and raising the funds necessary to activate our Crisis to Care Pilot and Beyond Stigma documentary film as well as fully mobilize our Directors, we are establishing the infrastructure required to dismantle the Neglect Tax.

By securing the Right to be Well, we are stopping the financial and human bleed of chronic crisis. The era of systemic abandonment is over. It is time to restore medical integrity to the American heart.
